Membership Fees

Covers routine pediatric primary care described in Services and detailed further under FAQs

Birth to 12 months

$175 / month

This includes up to 5 home visits to homes in the 27713 zip code during the first three months of life. 

12 months to 24 months

$150 / month

2 years to 6 years

$125 / month

6 years to 22 years

$85 / month

Family Maximum

$300 / month

Enrollment Fee

$75 / child

Family maximum of $150 (2 or more children)
